In order to request and receive assistance from Cherish Life:
- You must be a U.S. citizen or an eligible non-citizen (as determined on a case-by-case basis) and have a valid Social Security number.
- You must demonstrate financial need by completing the basic information form (REQUEST FORM).
- You must submit appropriate documentation to substantiate your needs. An affidavit self-certifying your need OR an affidavit from a credentialed medical provider will be required. The affidavit forms will be sent to you after the basic information form is reviewed and approved.
